Sara Gilman and Jeremy Larsen founded Coherence Associates Inc. (CAI), in 2012. Together they founded the company with a vision of providing the best possible treatment for clients, within a collaborative professional team environment. After 30 years as a mental health professional in solo practice, Sara wanted to bring other clinicians together that share her passion for helping clients find hope and healing through life’s challenges, by utilizing evidence-based interventions with compassion and integrity. Jeremy’s experience and training has brought together the business model to support the company’s vision of compassionate quality care and clinical excellence. Sara and Jeremy are dedicated to building a strong and inspiring work environment that empowers mental health professionals, while they facilitate the important help and healing to our clients.
The team at CAI is made up of licensed mental health professionals. Their work with first responders, military veterans, school professionals, performers in sports and the arts, children, teens, couples, and families, has built a positive reputation in the local community. CAI is known for providing compassionate, efficient, and professional care in a safe environment.
CAI’s experienced team members specialize in areas of performance enhancement, mental toughness and resiliency training, depression, anxiety, trauma, addictions, PTSD, childhood stress, ADD, and relationship issues. The CAI team frequently works with a client, and additional family members, in a synergistic counseling approach. Due to specialized training in therapeutic modalities, such as EMDR Therapy, Therapists in the community frequently refer their clients to CAI clinicians to help them in producing efficient results in a collaborative fashion, maintaining the primary counseling relationship with the referring therapist.
As an accomplished athlete herself, Sara has worked with other athletes in peak performance training, helping them to increase their abilities as well as eliminate the “yips” (hesitations) that can prevent consistent and optimal peak performance. This work has expanded and led CAI counselors to excel in helping high level performers; athletes, musicians, artists, speakers, and business executives, to expand their potential in mind, body and spirit.
Sara and Jeremy are launching a second company, which will work closely with CAI, providing education, training, speaking, and consulting, to individuals and groups, on a range of topics, such as; first responder resilience, military trauma, growing through grief, domestic violence, substance abuse, posttraumatic growth, enhancing mental toughness, peak performance, health care management, and self-care for health providers.

Our counseling professionals are trained in evidence-based practices, providing the most efficient and effective methods for lasting change. Including EMDR, Hypnosis, CBT, Brainspotting, and HeartMath, Critical Incident Stress Management (CISM), along with other therapies. We address issues of stress, anxiety, depression, grief, addiction, trauma, PTSD, relationship challenges, childhood stress, peak performance, and business consulting. Sara G. Gilman, PsyD, LMFT, CEO, Clinical Director
Dr. Gilman is the Co-Founder and President of Coherence Associates, Inc. With their shared vision of providing a working environment where fellow therapists could thrive, she founded the company with Jeremy Larsen. As CAI’s Clinical Director, Sara supervises and inspires the clinical associates. She is a nationally recognized featured speaker on radio, TV and Keynote addresses at conferences, addressing issues of stress, trauma, addiction, self-care, and peak performance. As a licensed Marriage and Family Therapist, Sara specializes in the areas of performance enhancement, trauma and addictions.
In 2017, Sara graduated, magna cum laude, with her Doctorate Degree in Clinical Psychology, with a special certification in Sports & Performance Psychology, from California Southern University. Her dissertation focused on, ‘First Responder Cumulative Traumatic Stress Exposure; EMDR as an Intervention.’ In 1983 she obtained a Master of Science degree in Clinical Psychology. Due to her extensive work in the areas of critical incident stress management, she was awarded fellowship status with the American Academy of Experts in Traumatic Stress. In furthering her experience and training she has obtained certifications in; forensic traumatology Neuro-linguistic Programming-Master Practitioner, Master Hypnotherapist, Professional Executive Coach, HeartMath Practitioner, EMDR Therapy Consultant, and is past-president of the EMDR International Association. As a former firefighter/EMT, she is Critical Incident Stress Management (CISM)-certified, and has a passion for working with first responders and supporting agencies with their Peer Support Teams.
Additionally, Sara has long held a unique talent for working with athletes and other top performers, improving their peak performance and strengthening mental toughness through evidence based treatments in sports and performance psychology. She has worked with Olympic level and other elite athletes competing in triathlons, martial arts, gymnastics, baseball, cycling, swimming, basketball, and soccer. Jeremy Larsen, BA, CATC-III, Manager – Business Development
Is the Co-Founder and Operations and Business Development Manager for Coherence Associates, Inc. He obtained his Associates degrees in Alcohol and Other Drug Studies and Accounting in 2013, his Bachelor’s Degree in Psychology in 2016, is pursuing graduate degrees in business and psychology. He is a certified California Addiction Treatment Counselor (CATC-III), and has worked in residential and outpatient treatment programs.
Jeremy has a personal passion for helping those who suffer from addiction, and a strong working knowledge of the most current evidence-based practices in the mental health and addiction fields. In collaboration with Sara Gilman, he designed and implemented an outpatient recovery program called “Journey to Wholeness” for those suffering from the co-occurring disorders of trauma and substance abuse; the program is offered exclusively through CAI. Jeremy’s ongoing academic studies focus on business and mental health to continually provide the best possible atmosphere of excellence for our clinical staff and clients. He provides guidance and direction for the growth of the business, as well as, clinical and logistical support for the CAI Team. Reannon Kerwood, LMFT, Clinical Associate, Program Coordinator
Reannon has a Master’s degree in psychology, and has obtained additional specialty certifications in Domestic Violence/Sexual Assault Crisis Intervention, Anger Management, Critical Incident Stress Management (CISM), Eye Movement Desensitization and Reprocessing (EMDR), and Advanced Cognitive Behavioral Therapy (CBT). She is a clinical member of the California Association of Marriage and Family Therapists, International Association of Trauma Professionals, and San Diego Trauma Recovery Network. Reannon thrives on speaking to groups in the community. She enlightens her audience by teaching the fundamentals of stress management, building resilience, effective communication skills and active self-compassion.
Reannon works passionately and effectively with her clients to empower change, and move through transitional phases in life with grace, while helping to restore joy. Her compassionate nature and efficient style assists people to heal and grow, even if they have experienced significant life events, such as; death of a loved one, car accidents, assaults, abuse, health challenges, or relationship challenges. She has an innate ability to teach skills that significantly improves how someone walks through life. She works with adults and teens who may be challenged with fears, anxiety, depression, obsessive compulsive disorder, prolonged job stress, relationship problems, anger issues, sexual assault and trauma, and transitions into adulthood. Reannon is experienced at working with the whole family, whether they are struggling with parenting issues, recovering from deployments, or dealing with medical issues such as traumatic brain injury or chronic immune disorders. Reannon understands how life can throw unexpected curve balls and roadblocks in to anyone’s life. Connie Glenn, MS, LMFT, Clinical Associate
Connie is a Licensed Marriage and Family Therapist with a Master’s degree in Counseling. She brings a wealth of experience and knowledge to the CAI team as an educator for two decades. Following her love of teaching and working with families, Connie embarked on her clinical counseling journey to fulfill her dream of helping adults and children through the psychological challenges in life. Her well rounded training, compassion, and bright spirit brings hope and growth to the clients she works with. With confidence and care she helps people process through old hurts, cultivate connections, let go of unwanted tendencies, and develop new tools for managing their emotions in day to day living. Connie is a certified EMDR therapist through Trauma Recovery/EMDR Humanitarian Assistance Programs, as well as a trained SafeZones LGBTQIA Ally through San Diego State University. Her work is motivated by her passion for self-growth and a belief in the power of human potential, as she guides her clients on their journey of healing and thriving. Katie Militello, LMFT, Clinical Associate
Is a Marriage and Family Therapist Registered Intern and a Professional Clinical Counselor Registered Intern (supervised by Sara Gilman, LMFT), with a Master’s degree in Counseling Psychology. Katie’s specializes in life’s transitions of grief and loss particularly child and perinatal loss, launching teenagers to college, working with families and adolescents adjusting to divorce and blended families, couples adjusting to life with a newborn, pregnant mothers dealing with postpartum depression, and individuals adjusting to retirement or a job layoff. Katie’s previous counseling experience has included children’s mental health, juvenile diversion, bereavement, and school based as well as support group facilitation. Katie is simultaneously interning in the Carlsbad school district as a GUIDE (Greater Understanding through Intervention, Diversion and Education) counselor and the UCSD Maternal Health Clinic. Katie has completed basic training in EMDR through the Trauma Recovery/EMDR Humanitarian Assistance Program. She has also received extensive training in Bowen Family Systems Therapy.
Katie is a member of the American Association of Marriage and Family Therapists, as well as the California Association of Marriage and Family Therapists. Katie sits on the board of the San Diego-North County CAMFT chapter in Carlsbad, and serves as their Pre-Licensed/Team 3000 representative. Katie’s previous career was as a marketing consultant working with corporations and family owned businesses to help them achieve their goals and become more successful. She found that her work in business fell short of accomplishing her goals of seeing her clients achieve their best possible potential; consequently, she was drawn to learning and understanding interventions, therapies and the field of counseling to provide support to improve the lives of others. Josh Garcia, LCSW, Clinical Associate
Josh is a Licensed Clinical Social Worker (LCSW), with a Master’s Degree in Clinical Social Work, and a BA in Psychology. He has completed basic training in Eye Movement Desensitization and Reprocessing (EMDR Therapy) with the EMDR Humanitarian Assistance Program. Josh utilizes EMDR Therapy, Cognitive Behavioral Therapy, and Dialectical Behavioral Therapy in assisting adults, families, and youth persevere through crises and various phase-of-life issues. Areas of focus include trauma recovery, anxiety disorders, depression, family conflict, and peri-partum depression.
As a former Psychiatric Emergency Response Team (PERT) Clinician with the Carlsbad Police Department and San Marcos Sheriff’s Department (2010-2014), he has experience working alongside Law Enforcement and other First Responders in providing critical clinical interventions and support. Experiences with the PERT Team have instilled the importance in recognizing and treating trauma in the community, in order to help those most in need, and to preserve public safety. Furthermore, experience with the PERT Team underscored the need to provide appropriate clinical support to the First Responders that keep local communities safe.
Josh concurrently works with Kaiser Permanente in providing therapy, psychosocial assessments, and group facilitation. He is also working toward completion of training as a Certified Disaster Mental Health Response Specialist (CDMHRS). Additional clinical experience includes serving as a PsyCare Therapist between 2009-2017, and as a former County of San Diego Qualified Mental Health Professional at the Polinsky Children’s Center Day Rehab Program from 2005-2010. Prior to this assignment, Josh worked with the County of San Diego as a Protective Services Worker in the American Indian Specialty Unit from 2000-2005. During his Master’s Degree training, Josh interned with the Escondido Youth Encounter STEPS Program for trauma-reactive youth, and the Oceanside Family to Family Unit. These experiences have lead to cultural competence, respect, open-mindedness, compassion and the appropriate use of humor as cornerstones to Josh’s work. Mauricio “Moe” Perdomo, MA, Associate Marriage & Family Therapist
Moe is a Registered Associate Marriage and Family Therapist (supervised by Sara Gilman, PsyD, LMFT) with a Master’s degree in Marriage and Family Therapy. His passion is to help individuals, couples and families know personal and relational healing. Having been through his own hardships in life, he believes we all have unique life experiences that shape who we are and how we connect with others. Sometimes those experiences create roadblocks that gets in the way of our ability to connect with those we love, in the way we desire. Moe understands that healing can be a scary or uncertain process, and that we do not have to go it alone. His years of training and experience as a counselor has affirmed his belief that humans are resilient and through finding self-compassion, relationships can heal more easily.
Moe believes it is important to respect what makes individuals uniquely “themselves” and to honor the story that is theirs, and theirs alone, while taking courageous steps towards personal healing and growth. He believes that those who have experienced traumatic or distressing life events need a secure and confidential place to heal, which generates a deeper experience of peace and joy. His wide range of training and experience, along with his care and compassion, empowers children, adolescents, individuals, couples and families to not only reach their goals, but to also to create a more meaningful life.
Working for many years within the school system, Moe saw a lot of stress, strain and anxiety that the kids were experiencing. He realized quickly that his work was not only with kids, teens, and young adults, but also with the entire family. Being a husband and father himself, Moe understands the importance of working with parents, individually and as a couple, to provide the family the best chance at sustained healing and personal growth.
Moe is trained in EMDR through Trauma Recovery/EMDR Humanitarian Assistance Programs, as well as Trauma-Focused Cognitive-Behavioral Therapy through the Medical University of South Carolina. He has had training and experience in Child-Centered Play Therapy with a registered play therapist. He attended The Daring Way National Training as a way to support clients in areas of shame, vulnerability and resiliency. Moe is a bilingual therapist who has experience working with both English and Spanish speaking children, adolescents, young adults, men, individuals, couples and families in the areas of trauma, childhood abuse, substance abuse, depression, anxiety, and family conflict. Kristen Garcia, MA, Associate Marriage & Family Therapist
Kristen is a Registered Associate Marriage & Family Therapist (supervised by Sara Gilman, PsyD, LMFT) with a Master’s degree in Marriage & Family Therapy. Kristen has extensive experience working with youth, young adults, and families, including teen girls on probation. A long side her clinical work with CAI, she is working with San Diego Youth Services at Kearny Mesa Detention Facility, where she facilitates prevention and intervention groups for commercial sexual exploitation. Kristen experiences a deep sense of satisfaction in helping these young women emerge from their traumatic experiences, to find a greater sense of esteem and life purpose.
In addition to her years of experience and training in a variety of therapeutic modalities, Kristen completed her EMDR training, through Trauma Recovery/EMDR Humanitarian Assistance Programs. Her calm, easy going nature helps her clients feel at ease, while she guides them through self-discovery, healing, and transformative personal growth. She is passionate about partnering with her clients in journeying toward more self-compassion, self-understanding, and healing. R. Scott Turner, MA, Associate Marriage & Family Therapist
Scott is a Registered Associate Marriage & Family Therapist (supervised by Sara Gilman, PsyD, LMFT), with a Master’s degree in Marriage & Family Therapy. As a former middle school teacher, he is able to bring his experience into counseling work with youth and their families, that now spans over 9 years. Through years of working with Vista Hill Learning Assistance Center, he worked collaboratively with teachers, parents and school staff to help children, ages 5-18, meet their goals and improve their overall life performance. Additionally, Scott worked for Casa Pacifica, Oceanside, where he counseled chronically mentally ill clients with co-occurring substance abuse disorders using cognitive-behavioral techniques, motivational interviewing, mindfulness, and dialectical-behavioral techniques, to facilitate and maintain recovery from mental illness and substance abuse disorders. His training and experience in a variety of settings, is a compliment to his kind, patient, and encouraging spirit.
Scott knows the effort it takes when faced with sudden change, and meeting new challenges, when he was invited in 2014, to be the co-writer of Afoot and Afield in San Diego County, 5th Edition, by Wilderness Press. This project launched him into conducting fieldwork, revising text, and documenting new hiking routes throughout San Diego County for this bestselling travel guide.
